Becoming a CHIP Affiliate

CHIP is charged by the University with helping to develop and further the research enterprise in the areas of health behavior, health behavior change, health intervention, and prevention at UConn, and with contributing broadly to these fields of inquiry. CHIP accomplishes these ends, in part, by supporting researchers in these domains.

After installation, download the appropriate form above by right clicking on the link and selecting “download linked file as”, then use Adobe Reader for Mac to open the form.
