Request for Space at CHIP

Request for Room Reservation

CHIP has a number of rooms that may be used by CHIP affiliates for running participants or for holding research-related meetings. To request a room reservation, please follow these steps:

1. Check availability of rooms.
• CHIP Room Availability
2. Complete appropriate form below and email to Donna Hawkins.
• If you are requesting Research Space in CHIP to Run Participants, please download and complete the Request for Research Space form.
• If you are requesting Space in CHIP to Run a Research-Related Meeting or a Meeting in Support of Research, please download and complete the Request for Meeting Space form.

Request for Long-Term Assignment of Research Office Space at CHIP

If you are requesting a long-term assignment of research office space at CHIP, or requesting a renewal of long-term research office space at CHIP, please review the CHIP Research Office Use Guidelines and complete the Request for Long-Term Research Office Space at CHIP form and submit it to Stacey Leeds.
